I think this is the only time I have ever seen this 'error'.

It is perfectly legitimate. Several times in the late 1960s, early
1970s Mint workers fed coils of planchet strip intended for one
denomination into a blanking press set up for another denomination.
Thus, you got thick dimes or thin quarters, the most common one being
the 1970-D thin quarter. A 1969 thin quarter is much scarcer. They made
the papers when a batch of thick dimes got released into circulation
in, IIRC, Kansas City, and they jammed all of the pay telephones in
town.
Remember pay telephones?
